What's the Catch?

You are responsible for costs not reimbursable by Medicare, such as co-payments and deductibles.

Advice

If you are a Medicare beneficiary and have diabetes, the cost of your testing supplies may be covered, whether you use insulin or not. Be sure to check out Liberty's site for more information.

FAQs

	How soon can I start using the Diabetes and Respiratory Supplies benefit?
	You can place your first order upon receiving your Everyday HealthCard enrollment confirmation e-mail, which will contain your membership ID #. and allow you access to the member site.
	What is a HbA1c test?
	The A1c test is a measure of a person’s total glycated hemoglobin (HbA1c) and reflects their average blood glucose (sugar) content for the previous 2 – 3 months.
	Are there any up-front out-of pocket expenses when using this service?
	Other than co-payments and deductibles, You pay no up-front out-of pocket expense.* If you have qualified supplemental insurance, you may owe nothing at all.
