Simply search our online databases or call the toll free number listed in your online membership guide for a provider in your area.

Search our online database:

Show your membership card to the provider at the time of your visit to receive the discount and identify your self as an Everyday HealthCard Program member. Call the number on the back of your card for more information.

What's the Catch?

Beltone products are available only through authorized Beltone Hearing Aid Centers.

Be sure to identify yourself as a member of the Everyday HealthCard Program and present your card during your visit.

FAQs

	How soon can I start using the Beltone Hearing Care discount?
	You can start looking for a participating authorized Beltone provider upon receiving your Everyday HealthCard enrollment confirmation e-mail, which will contain your membership ID #. Allow 15 days to receive your membership card which will be needed to actually obtain your discount.
	Will I always get a discount?
	Yes, you will receive a 15% member discount at all participating providers.
